This service is for Shoreline Employees to request to checkout hardware from Shoreline Technology Support Services.

Technology Support Services Equipment Checkout Request Form

By checking out the equipment described below, I, the borrower, agree to the following terms and conditions:  

  1. Equipment Return:  
    1. I will return the equipment, including packaging, manuals, and all associated materials, in complete and undamaged condition to Shoreline Community College Technology Support Services.  
    2. The return should occur at the end of my employment or when the device is no longer needed.  
  2. Additional Criteria for Laptop Checkouts:  
    1. Full-Time Employees:  
      1. If I choose a laptop in lieu of a desktop, I will return it at the end of my employment.  
      2. If I have a desktop and need a short-term loaner laptop, I will return or renew it at the end of each quarter.  
    2. Part-Time Employees (including Contractors):  
      1. If I choose a laptop in lieu of a desktop, I will return it at the end of my contract or employment.  
      2. If a desktop has been provided but I need a short-term loaner laptop, I will return or renew it at the end of each quarter.  
  3. Usage Guidelines:  
    1. I will use the equipment and associated college-provided consumables (such as toner and paper) strictly in accordance with the college's Policies/Procedures.  
    2. I will not use the equipment for personal, commercial, or political purposes.  
    3. I am bound by the Faculty & Staff Acceptable IT Use Policy and the Faculty & Staff Acceptable IT Use Procedure.  
  4. Ownership and Prompt Return:  
    1. I understand that the equipment remains the property of Shoreline Community College.  
    2. I will promptly return the equipment upon separation of employment from the college, at the end of each quarter, or if requested by the college administration.  
  5. Inspection and Inventory Verification:  
    1. I acknowledge that the college may require the equipment to be returned at any time for update, inspection, or inventory verification.
