Employees with permanent or temporary disabilities have the right to reasonable accommodations under Title I of the Americans with Disabilities Act (ADA). Shoreline is committed to working with employees with disabilities to identify and implement reasonable accommodations as appropriate for each individuals' needs.
To request an accommodation, please complete the Employee Accommodation Request Form. A member of the Human Resources team will review your request and reach out to you within three (3) business days to discuss the next steps in the accommodation process.
Here is what you can expect once HR has reviewed your request:
- The HR accommodations specialist will send you a Medical Certification Form to take to your provider along with a copy of your job description. The form asks for specific information about your work restrictions relative to your disability and, if the disability is temporary, the duration of the restrictions.
- Once your provider returns the completed Medical Certification Form, the HR accommodations specialist will reach out to you to schedule a meeting to start the conversation about appropriate accommodation options to meet your specific medical needs. Possible accommodations may include assistive technology, ergonomic equipment, schedule modifications, etc.
- The HR accommodations specialist will notify you of any approved accommodation and provide a timeline for putting the accommodation in place.
- Once the accommodation is in place, the HR accommodations specialist will follow up with you to determine if additional modifications are needed.
