When hiring hourly or student employees, please use the New Hire Packet which includes an updated fillable PDF Personal Action Form (PA). The packet also includes directions for activating their ctcLink account, setting up their direct deposit, and completing their covid vaccination attestation. Please remember that the PA must be approved by the budget office and assigned a combo code PRIOR to sending the packet to HR for processing.
Documents Needed:
- 2022 Personal Action Form (PA) signed by the employee, supervisor, and budget office
- Employment Form for Hourly/Student Employees
- W-4
- I-9 -
- This needs to be signed by the employee and Shoreline staff in person.
- On pg 2, be sure to write "N/A" on all of the spaces in lists A, B, and C that you do not use, and whoever completes the document with the employee must sign off and complete the "Certification" section under the list section- if you complete the form electronically, the n/a's will autofill but you still need to print and sign it
- Copy of Social Security Card (yes, we need this one for payroll regardless of what was used for the I-9)
- Retirement Status Form
- Federal & State Reporting
- Declaration and Authorization to release information
- Substantial Presence Test (Non-Resident Aliens Only)
- Parent/School Authorization (only if under 18yrs old)
- Copy of the Covid shot record or exemption- they WILL need to bring the physical record to the HR office for verification as soon as they are on campus
When completed, the packet can be dropped off in the office or emailed to hr@shoreline.edu.
