How to Create Forms

Personally Identifiable Information (PII)

When creating/editing forms, do not request any Personally Identifiable Information (PII) such as date of birth, social security number, drivers license number, national identification number, passport number, financial information, etc.

Create a Form

  1. Create Forms in the Sandbox environment first.
  2. After you log into TDX, click on the tab of the application you want to add a form to. 
  3. Click the gear icon in the upper right side and select Admin. If you do not have Admin access, contact the TDX administrator.

  4. Click on Forms on the left hand side of your screen.

  1. You will now see all the Forms available for that Application.
  2. To create a new Form, click the New button at the top of the page.
    • Add Name.
    • In Classification, select Service Request.
    • Select Active, then the Form will close.
    • Go to the Form and click Edit.
    • Click the Designer tab.
    • Copy and paste the name in Title. 
    • Always include the three fields shown below and set them up in the same way.

  • Select Editable and Required to make the field mandatory. 
  • Add the fields you want. You may want to look at other forms as reference.
  1. To edit an existing one, double click the Form or click Edit to make changes.
  2. To delete, click delete on the right side of the page. If you are unable to delete the form, send a request to TSS.

Add a Form to a Service Request

  1. Go to the Service location you want to add a Service Request to.
  2. Click New Service.
  3. Copy and paste the Title into Name.
  4. Add a Long Description like this- To request NAME OF SERVICE REQUEST, click on the Request Service button on the right hand side of the screen and fill out the form.
  5. Add Request Application.
  6. Add Request Type.
  7. Add tags.
  8. Click the Save button.
  9. Click the Edit Service button.
  10. Click Form at the top of the screen.
  11. Click Use an existing Shared Form.
  12. Select the Form you want to use.
  13. Click Save. 

Allow Public Requestors (no login required)

  1. In the Service Request, select Edit Service.
  2. Click Forms
  3. Check the box Allow Public Requestors.
  4. Add Shoreline Community College as the Default Acct/Dept.
  5. Click Save.
