Create Student Feedback Forms

Create new Student Feedback Forms at the beginning of each year. Use the same form for the entire academic year.

Create a Report

  1. Log in to Survey Monkey. 
  2. Click on My Surveys at the top of the screen.

  1. Click on the survey you need a report for. 
  2. Click Analyze Results.

  1. Click Individual Responses.

  1. Click the Save As button.
  2. Select Export File.
  3. Select All Response Data.

Save as drop down menu. There are red squares around the save as, export files, and all responses data buttons.

  1. The file will download to your computer.
  2. Open the file.
  3. Click Enable Editing.
  4. Select all and open up all the cells.
  5. Cut/paste the relevant cells from Row 1 to Row 2.
  6. Delete the top row.
  7. Add filters.
  8. Select the dates needed. 
  9. Delete extra/empty columns.
  10. Select the instructor needed.
  11. Save file using this naming convention Year Quarter Instructor Name.
  12. Email the file to whoever asked for the results.

Disaggregating data for ARC graphs in Survey Monkey 

  1. Log in to Survey Monkey.  

  1. Click on Home on the top of your screen.  

  1. Choose the survey you need results for and click the Analyze icon

  2. Click +FILTER under the Rules tab on the far left.

  1. Click Filter by Question and Answer on the drop-down menu.

  1. Select the question you want to sort by.  

  2. Select whose data you want to view and click the green APPLY button at the bottom. This will remove all data that is not correlated to a response for the person you selected.

  1. Click the green SAVE AS drop-down menu.  

  2. Click the EXPORT FILE drop down menu.  

  3. Select All summary data

Save as drop down menu. There are red squares around the save as, export files, and all responses data buttons.

  1. Change the file format to XLS (Excel).

  1. Click the optional setting to include open-ended questions. 

  2. Click Export. The upper right-hand corner will have a pop up to indicate when the export has started and completed.  

  1.  There will be a secondary pop up with a green Download button. If you miss the pop up can see all your recently exported files on the left side tab.

  1. Once you are done, delete the +FILTER rule you created by clicking the ellipses next to your filter request. 

  2. Click delete rule on the drop-down menu.

  1. Click the orange delete and the survey will return to its original state.

  1. Email the file to whoever asked for the graph.
