Student Feedback Forms

Quarterly student feedback forms (SFF/SF) are administered by Learning Resources using Watermark Course Evaluations & Surveys (CES). SFF are anonymous and instructors do not receive results until the day after grades are due. Non-instructional SFF are administered by Learning Resources using Survey Monkey. 

The process was designed with direction from the Office for Student Learning and Success, the Faculty Union, and the Office of Institutional Effectiveness and Data Management (IADM). The confidential feedback from students help improve student learning and is critical to faculty development and program planning and improvement. 

Observations are conducted by administration and faculty; these are used in part to form an ARC. SFF in this article are student feedback on courses or interactions with specific non-instructional faculty. Previously these were referred to as Student Evaluations, per the updated CBA effective July 1st, 2024, the same set of forms is referred to as "student feedback". 

Who is Required to Receive SF?

SF requirements are outlined in the AFT faculty contract. 

How Often are SF Done?

The frequency of the SF is determined by the AFT faculty contract and depends on whether faculty are tenure track, tenured, new associate faculty, on the Priority Associate track, or already a Priority Associate. 

SF Dates

Student feedback forms will be available to students during the quarter after the seventh week and for at least seven (7) consecutive calendar days. Within those parameters, each faculty member will determine when and for what length of time student feedback forms will be issued, not to extend past the last day of instruction.

The Forms

Student have access to either the Main Survey or a form specific to ESL and Parent Educators. Librarians, media techs, counselors, and advisors are evaluated using Non-instruction Evaluation Forms (P, Q, W, X, Y).

Survey Monkey Forms

• Instructors will provide a link or QR code to the student.

Access to Results

CES SFF

• Division designees can access results the day after instruction ends in order to provide reports for required faculty to supervisors. For more information and instructions, view Course Evaluations & Surveys (CES) for CES Administrators.
