What is the procedure for individual clock hour certificates?

For each participant wanting to receive clock hours, the student submits the $15.00 Professional Development Clock Hour Fee form.
After processing the fee, send the student the
Continuing Education Clock Hour Inservice Registration Form
.
Have the student fill out the form and sign it. Include this information for section II in your reply:
The course name, quarter/year, and, item number
Include first and last day of quarter
Number of credit hours of class. (One quarter hour of college credit is the equivalent of 10 clock hours for a total of 50 clock hours)
Shoreline Community College is the provider and our address is 16101 Greenwood Ave. N, Shoreline WA 98133
Name of instructor
They will need to scan the signed form and send it back to continuing-ed@Shoreline.edu.
When the signed form is returned, print it out and have Dawn Lowe-Wincentsen review and sign it.
Scan the completed signed form and send it back to the student.
